Special Transportation Permit - Oversize Load

To transport wide loads or oversize loads on City streets, a wide or oversized load permit is required. You may apply for this permit by using the PDF form (below) or you may apply, in person, in the Traffic Division of the Engineering Department at City Hall. If you have any questions please contact us during our normal work hours of 8:00 a.m. to 5:00 p.m. Monday through Friday (holidays excepted) at (408) 615-3000.
 
This PDF form is not interactive, due to the requirement for an original signature, but it may be printed, filled out, and mailed, faxed or delivered to the City.
 
To obtain a permit using this form you must complete the following:
 

1. Print the permit application form Special Transportation Permit (PDF).
2. Fill out the permit application form completely. Include authorized agent signature.
3. Mail, fax (see "Fax" above) or deliver the completed permit application form, with payment to:

Allow at least two working days to process your request for a wide or oversize load permit. Failure to provide complete information may delay issuance of the requested permit.

 
Fax: Faxing service is available to businesses in good standing with the City of Santa Clara. Faxed permit forms must be accompanied by proof of payment, which may be in the form of a photocopy of the signed check made payable to the City of Santa Clara. To remain in good standing, all checks for payment of wide or oversized load permits must be received by the City of Santa Clara within ten (10) working days of the faxed request.
