3. Provide proof of Santa Clara residency
4. Return completed packet to the YAC

The following Participant Guidelines, Participant Behavior Standard, and Discipline Plan have been prepared to help your child experience a successful and enjoyable time at the Youth Activity Center, Teen Center and Skate Park.
