Billing requests or inquiries for ambulance transports provided by the City of Santa Clara Fire Department can be requested by email at fire@santaclaraca.gov
Please provide the patient’s name, date of service and incident or report number provided at the time of transport. You may be asked to complete the Authorization for Medical Release form, depending on the request. These can be requested along with the PCR request.
