The Health & Wellness Program at the Santa Clara Senior Center connects residents age 50+ with tools, guidance, and support to help them stay healthy and independent. Our registered nurses offer personalized health education, wellness check-ins, care planning, and referrals to trusted community resources.
From informative webinars and monthly health mailings to in-person screenings and one-on-one consultations, we’re here to help you navigate your wellness journey with confidence - no matter where you are in life.
Weekly Wellness
Mindfulness Meditation
- First Tuesday of the month at 10:15 a.m. via Zoom with Chris Becker.
- All other Tuesdays in person at the Senior Center, room 231

Thursdays, 10 - 11 a.m. | Senior Center, room 131.

Health Education and Coaching
Our RNs teach and facilitate activities about important health-related concerns. They also provide information and referral to services/resources such as Access VTA, transportation, food assistance, utility discounts, home care, insurance, housing, and more:
- Elder abuse concerns - Caregiver support for families
- Chronic disease management - Advanced Healthcare Directives & POLSTs
- Monthly wellness checks via phone - Navigating your healthcare system
- Fall prevention and home safety - Making appointments
Care Management
Enrollment as a care management client follows an intake interview. This service is free and for City of Santa Clara residents and their families. We identify health, social, and functional needs through assessment, then develop a care plan. We help you access services and evaluate outcomes. We provide information and help you connect to community resources.
